I can't add anything to its scoutability that hasn't already be said, but I can give some info on running it low. A few of us did it last year at 230cfs, and I have pictures/video up here: http://picasaweb.google.com/tm.dubois/20090725LowWaterUpperGauley#
I can't speak for medium flows, but at 230cfs it was really fun, ELF, technical creeking. With a good guide and willingness to walk anything you don't like, class IV- skills should be adequate.
There are lots of sieves, but they are entirely avoidable as long as you always stay observant and generally go where the most water is. The sketchiest spots were toward the end of lost paddle, and iron ring (the only place most of us walked).
Insig. almost lives up to its name at that level, and sweets channelized beautifully.
