they've started by saying "oh, see you were a long-distance swimmer" because of the bone growth. The surfer's ear is a natural reaction to exposure to cold water. It's more of a concern now because boaters spend a lot more time (because of drysuits) down in cold water than we did years ago. In the early days we boated on the water, not in it. In later years I've been wearing a beanie with ear flaps under the helmet. A divers hood works very well also. Sometimes I layered both pieces of head gear and didn't even need ear plugs. Kept a slightly larger helmet for winter use.

Swimmer's ear is an infection that results from frequent exposure to water (doesn't have to be polluted water). The ear doesn't every quite get dry which changes the pH and makes a great place for bacteria to grow. Use a commercial product ("Swimmers Ear" or the vinegar solution when you get off the river especially in the summer months.
